About the role
We are seeking an experienced Care Coordinator to join a well‑established domiciliary care provider in Leeds. This full‑time, permanent position offers a dynamic, hands‑on environment where you will support the delivery of high‑quality care to service users across the region.
Key responsibilities
- Produce and manage staff rotas to ensure all care calls are covered.
- Liaise with hospitals, GPs, social services and families to support service users.
- Complete care assessments and reviews, maintaining high standards of care.
- Participate in and manage the on‑call rota.
- Provide flexible support to colleagues and other departments as needed.
Required profile
- Previous experience in a domiciliary care setting.
- Full UK driving licence and access to your own vehicle.
- Ability to work in a fast‑paced, flexible environment.
- Strong organisational and communication skills.
