Care Team Leader – Heath Farm Residential Services

About the role

Lifeways Group is seeking an experienced Care Team Leader to join the Heath Farm residential services team. You will support adults with learning disabilities, complex needs, mental health conditions and challenging behaviours, ensuring they live fulfilling, independent lives.

Key responsibilities

• Lead and motivate a team of support workers to deliver high‑quality, person‑centred care.
• Oversee recruitment, staff supervisions, interviews and ongoing development of team members.
• Maintain accurate records and documentation using strong written and IT skills.
• Communicate effectively with staff, service users and external professionals.
• Support individuals to achieve personal goals and promote independence.

Required profile

• Qualified with an NVQ/QCF in Health & Social Care (or equivalent) or substantial experience as a Care Assistant or Support Worker.
• Demonstrated commitment to delivering compassionate care and a desire to progress into leadership.
• Ability to manage a team, handle recruitment processes and provide effective supervision.

Required skills

• Strong written communication and IT skills for accurate record keeping.
