Care Worker / Support Worker – Client‑Focused Role

About the role

This position is based in Northolt and provides client‑focused care and support within a day service and vocational setting. You will work closely with clients, their families and multi‑agency partners to help individuals achieve personal goals while maintaining dignity, privacy and independence.

Key responsibilities

• Deliver high‑level, person‑centred care and support in line with each client’s expressed wishes and health needs.
• Safely handle, dispense, administer and record medication where required.
• Design and assist with outcome‑based activities that promote safety, independence and community participation.
• Support clients to work towards agreed personal outcomes, recognising strengths, hobbies and skills.
• Maintain accurate, timely records and communicate effectively with families, carers and health‑social care professionals.
• Participate in multi‑agency meetings and collaborate with staff to share best practice.

Required profile

• Commitment to respecting client privacy, dignity and the right to be involved in their own care.
• Ability to work flexibly, including evenings and weekends, to meet client needs.
• Strong communication skills with clients, families and professional partners.
• Willingness to promote client independence and healthy lifestyle choices.

Required skills

• Safe handling, dispensing and administration of medication.
• Accurate record‑keeping and documentation.
• Ability to plan and evaluate person‑centred activities.
