Clinical Pharmacist

About the role

This position offers a fantastic opportunity to join the pharmacy team at Brooklands Medical Practice, supporting medicines optimisation across primary‑care services. You will work closely with a multidisciplinary clinical team to ensure safe, effective prescribing for the local community.

Key responsibilities

• Handle prescription‑related queries from patients and monitor medication use for long‑term conditions.
• Review clinical letters and hospital discharge summaries, implementing appropriate medication changes.
• Conduct clinical medication reviews focusing on polypharmacy, frailty and multimorbidity.
• Collaborate with the prescribing lead and ICB medicines management team to promote evidence‑based, cost‑effective prescribing.
• Support the wider clinical team and contribute to teaching and professional development activities.

Required profile

• Registered Pharmacist with at least two years post‑registration clinical experience.
• Membership of the General Pharmaceutical Council or Royal Pharmaceutical Society.
• Understanding of general practice and primary‑care prescribing.
• Strong commitment to high‑quality, patient‑centred care.
• Excellent written and verbal communication skills.
• Willingness to engage in teaching and continuous professional development.

Required skills

• IT proficiency (keyboard and computer skills).

What we offer

• Part‑time permanent role (30 hours per week) with flexible working hours between 08:30‑18:30, Monday‑Friday.
• Pro‑rated salary ranging from £47,810 to £54,710 per year, dependent on experience.
• Membership of the NHS Pension Scheme.
• Up to 7.6 weeks annual leave (including bank holidays).
