Community Health Navigator (Full-Time)

About the role

The Community Health Navigator is the first point of contact for patients, the public and healthcare professionals accessing the Bath & North East Somerset, Swindon and Wiltshire Community Health Services Single Point of Access. You will guide service users to the right care, manage referrals and support clinicians in a fast‑moving environment.

Key responsibilities

• Answer inbound calls, emails and post with empathy, assess urgency and direct enquiries appropriately.
• Maintain confidential patient records and update key IT systems, including processing referrals and correspondence.
• Book, cancel and reschedule clinical appointments while meeting waiting‑time targets.
• Identify and escalate urgent or incomplete referrals to the relevant healthcare professionals.
• Provide administrative support to clinicians and managers to ensure smooth service delivery.
• Work flexibly within a supportive team, contributing to service priorities as needed.

Required profile

• Motivated and customer‑focused with excellent communication skills.
• Strong organisational ability and meticulous attention to detail.
• Comfortable handling sensitive and confidential information.
• Confident using IT systems and able to work effectively in a busy team.

Required skills

• Proficiency with patient record and referral IT systems.
