Community Health Navigator – Full-Time

About the role

We are seeking a motivated Community Health Navigator to join the Bath & North East Somerset (BSW) Community Health Services Single Point of Access (SPA) team. In this full‑time position you will be the first point of contact for patients, the public and health‑care professionals, helping them access the right services quickly and compassionately.

Key responsibilities

• Answer inbound calls, emails and letters, assess urgency and direct enquiries to the appropriate service.
• Maintain accurate patient records and update key IT systems, including processing referrals and correspondence.
• Book, cancel and reschedule clinical appointments while meeting waiting‑time targets.
• Identify and escalate incomplete or urgent referrals to clinical colleagues.
• Provide administrative support to clinicians and managers to ensure smooth service delivery.
• Work flexibly within a supportive team, contributing to service priorities as needed.

Required profile

• Strong customer focus with excellent verbal and written communication skills.
• High attention to detail and ability to handle confidential information securely.
• Organisational agility in a fast‑paced environment.
• Willingness to work on‑site in Bath and transition to remote work after probation.
