Community Health Navigator (Part‑Time)

About the role

We are seeking motivated, customer‑focused Community Health Navigators to join our Bath & North East Somerset, Swindon and Wiltshire (BSW) Community Health Services Single Point of Access (SPA) team. As the first point of contact, you will help patients and the public access the right care at the right time.

Key responsibilities

• Answer inbound calls, emails and post, assess urgency and direct enquiries to the appropriate service.
• Process referrals, update patient records and manage key IT systems securely.
• Book, cancel and reschedule clinical appointments while meeting waiting‑time targets.
• Identify and escalate urgent or incomplete referrals to clinical colleagues.
• Provide administrative support to clinicians and managers to ensure smooth service delivery.
• Work flexibly within a supportive team, contributing to service priorities as needed.

Required profile

• Excellent communication and organisational abilities.
• Strong attention to detail and ability to handle confidential information.
• Comfortable working in a fast‑paced environment and collaborating with clinical staff.
