Community Mental Health Nurse – 6‑month secondment

About the role

This 6‑month secondment offers a qualified Mental Health Nurse the chance to work as a Community Mental Health Practitioner within the Ealing Acton MINT service. You will join a multidisciplinary team, manage a caseload of adults and older adults with diverse mental health needs, and collaborate closely with local GP practices and community partners.

Key responsibilities

• Assess, plan and deliver care for patients with a range of mental health conditions.
• Provide brief, evidence‑based interventions tailored to individual needs.
• Coordinate with primary, secondary and voluntary services to ensure integrated, place‑based care.
• Maintain accurate records and contribute to multidisciplinary case discussions.
• Support the dissolution of barriers between mental health, physical health and social care.

Required profile

• Registered Mental Health Nurse with current NMC (or equivalent) registration.
• Experience working with varied mental health diagnoses across adult and older adult populations.
• Strong written and verbal communication skills to build relationships with local teams and partners.
• Ability to work autonomously while contributing to a collaborative team environment.

Required skills

• Current registration with the Nursing and Midwifery Council (NMC) or relevant governing body.
