Community Nurse – NHS Band 5

About the role

This permanent position as a Community Staff Nurse (Band 5) supports the delivery of high‑quality, person‑centred nursing care to adults and older people in their own homes and other community settings. You will work within a District Nursing team, collaborating with multidisciplinary partners to promote independence and prevent unnecessary hospital admissions.

Key responsibilities

• Deliver holistic nursing care to patients in community settings.
• Assess, plan, implement and evaluate individualised care plans.
• Manage a defined caseload, prioritising workload effectively.
• Collaborate with GPs, multidisciplinary teams and partner agencies.
• Provide health education, promote self‑care and maintain accurate records.
• Support students, new staff and junior colleagues.
• Participate in team meetings, audits and service‑improvement initiatives.

Required profile

• Registered Nurse with a current NMC registration.
• Accountable to the NMC Code of Conduct and Trust policies.
• Ability to work autonomously and recognise clinical risk.
• Commitment to continuous professional development and reflective practice.
