Dementia Care Manager

About the role

The Dementia Care Manager will lead the Dementia Community at Fleet Hall Care Home, implementing the company’s dementia care strategy and managing resources within budget. You will ensure person‑centred care plans reflect residents’ needs and preferences while fostering a supportive environment for both residents and their families.

Key responsibilities

• Oversee overall management of the Dementia Community and embed the dementia care strategy.
• Ensure daily care plans are accurate, integrated into person‑centred care summaries, and reflect resident and relative input.
• Build caring relationships with residents and families, acting as an advocate and managing conflict when needed.
• Collaborate with the Activities Team to develop meaningful activities for residents with dementia and set expectations for staff participation.
• Promote a culture that maximises residents’ independence, capabilities, and quality of life.

Required profile

• Minimum three years’ experience in a similar dementia care management role within a fast‑paced environment.
• Comprehensive understanding of CQC standards and requirements.
• Relevant industry qualifications.
• Knowledge of UK health and social care market and care services businesses.
