Discharge and Flow Coordinator

About the role

Join the Discharge and Flow team on Longleat Ward at Warminster Community Hospital and help patients leave safely and promptly. As a Band 3 Discharge and Flow Coordinator you will work closely with nurses, therapists, social workers and other multidisciplinary staff to keep patient flow moving efficiently.

Key responsibilities

• Coordinate patient discharge plans and ensure timely hand‑over to community services.
• Maintain accurate patient records, complete referrals and update discharge documentation.
• Communicate discharge information clearly to patients, families and the wider care team.
• Support multidisciplinary meetings and contribute to flow‑optimisation initiatives.

Required profile

• Motivated, compassionate and well‑organised.
• Strong interpersonal skills and ability to build supportive relationships with patients and families.
• Experience working in a hospital or community health setting is desirable.
