District Nurse (Band 6) – Cardigan & Newcastle Emlyn

About the role

This full‑time Band 6 District Nurse position supports patients in Cardigan and Newcastle Emlyn, delivering complex community care within the Hywel Dda University Health Board. The role requires a qualified practitioner to manage a delegated caseload and ensure high‑quality care in patients' homes or clinics.

Key responsibilities

• Provide high‑quality, complex care management to patients in their own homes.
• Liaise with patients, carers, staff and external agencies to coordinate care packages.
• Ensure appropriate delegation of duties, risk management and effective communication.
• Deputise for the Band 7 District Nurse Team Leader, covering operational, quality and resource management duties when required.
• Travel between Cardigan and Newcastle Emlyn sites to meet service demands.

Required profile

• Current employee of Hywel Dda University Health Board.
• Previous community nursing experience.
• Specialist Practitioner Qualification in District Nursing (SPQ).
• Ability to work 8 am‑8 pm, seven days a week, including weekends and bank holidays.
• Welsh language ability is desirable but not mandatory.
