Ex‑Offender Mentor – Community Justice Worker

About the role

Join a dedicated team in Swindon as an Ex‑Offender Mentor, supporting individuals as they transition back into the community. You will guide participants through assessment, planning and delivery of tailored support, helping them overcome barriers and achieve lasting change.

Key responsibilities

• Manage a caseload of ex‑offenders, conducting assessments and creating personalised support plans.
• Provide one‑to‑one mentoring, group interventions and practical guidance.
• Facilitate access to housing, mental‑health, substance‑misuse, education and training services.
• Collaborate with peer mentors, volunteers and partner agencies to secure resources.
• Maintain accurate case records and follow safeguarding and risk‑management procedures.
• Work towards individual and team performance targets and contribute to service improvement.

Required profile

• Empathetic, resilient and committed to helping vulnerable people.
• Experience in health and social care, mental‑health, youth work, housing, substance‑misuse or community outreach is desirable.
• Strong communication and relationship‑building skills.

Required skills

• Ability to assess needs and develop actionable support plans.
• Knowledge of local support services and referral pathways.
• Proficiency with case‑management systems and record‑keeping.
