Experienced Healthcare Assistant - Remote

About the role
We are looking for an experienced Healthcare Assistant to join a fully remote Hospital‑at‑Home service. You will support patients who are monitored at home using digital technology, helping to keep them safe while reducing unnecessary hospital admissions.
Key responsibilities
- Conduct scheduled discovery and wellbeing calls with patients.
- Verify patients are feeling well and taking medication as prescribed.
- Clear and manage green alerts according to protocol.
- Identify changes in condition and promptly escalate concerns to nurses or the clinical team.
- Submit updates and requests to the wider clinical team.
- Provide reassurance, clear communication and patient education.
- Accurately record interactions and observations in the system.
- Maintain confidentiality and adhere to data‑protection standards.
Required profile
- Previous experience as a Healthcare Assistant, preferably within the NHS.
- Confident telephone manner with strong communication skills.
- Good understanding of patient care, medication awareness and escalation pathways.
- Ability to work independently and manage workload remotely.
