Field Care Supervisor

About the role

We are looking for an experienced Field Care Supervisor to join our home‑care team. You will act as the vital link between our office, care staff, and clients, ensuring person‑centred, high‑quality care is delivered consistently.

Key responsibilities

• Conduct comprehensive initial assessments for new clients and develop tailored care plans.
• Create, review, and update detailed care plans and risk assessments.
• Perform regular competency and spot checks on care staff to maintain standards.
• Visit clients regularly to review care effectiveness, gather feedback, and adjust plans as needed.
• Liaise with families, GPs, district nurses, social workers, and other professionals to coordinate care.
• Participate in auditing activities and contribute to continuous improvement.

Required profile

• Proven experience in home‑care or community‑care supervision.
• Strong communication, organisational, and leadership abilities.
• Ability to assess client needs, develop care plans, and manage risk.
• Comfortable working as a bridge between office staff, care workers, and external professionals.
