Health Care Assistant – Community & Residential Care

About the role

As a Health Care Assistant you will provide safe, respectful and person‑centred care to residents and service users in both residential homes and community settings. You will work closely with another carer when required and deliver dedicated 1:1 support to allocated individuals.

Key responsibilities

• Assist with personal care tasks such as washing, dressing, toileting and continence support.
• Use moving and handling equipment safely, including hoists, transfer aids and wheelchairs.
• Provide 1:1 care, monitoring residents closely and responding to risks such as falls or challenging behaviour.
• Encourage meals, record food and fluid intake, and support safe mobility and pressure‑area care.
• Prompt medication when trained and authorised, and maintain accurate hourly care records.
• Report any changes in health, behaviour or safeguarding concerns to senior staff promptly.
• Deliver community‑based care, arriving punctually at service users’ homes and assisting with light household duties, shopping and companionship.

Required profile

• Commitment to delivering person‑centred care in line with individual care plans.
• Ability to work collaboratively with another carer and follow risk assessments.
• Strong observational skills and the capacity to act quickly to reduce risks.
• Willingness to travel between Bath, Bristol and Swindon locations.

Required skills

• Competence in using moving and handling equipment safely.
• Basic medication prompting knowledge (when trained).
• Accurate documentation and record‑keeping abilities.
