Health Visitor – Community Family Support (Full/Part Time)

About the role
Join a highly rated community health team delivering outstanding care across South Bedfordshire and Luton. As a Health Visitor you will work directly with families, providing home‑based support and coordinating integrated services to improve health outcomes for children and young people.
Key responsibilities
- Conduct home visits to assess and support the health and wellbeing of families.
- Partner with families to co‑produce care plans and ensure seamless referral to specialist services.
- Utilise new digital initiatives to enhance service delivery and record keeping.
- Contribute to the team’s continuous improvement and innovation agenda.
Required profile
- Qualified Health Visitor, either newly qualified or with up to several years of experience.
- Commitment to high‑quality, family‑centred care.
- Willingness to work across multiple locations in South Bedfordshire and Luton.
