Healthcare Assistant – Hospital at Home

About the role

The Hospital at Home service is looking for a compassionate Health Care Support Worker to deliver person‑centred care to patients in their own homes. You will join a supportive community nursing team, helping to avoid admissions and support early discharge while working in a fast‑paced, varied environment.

Key responsibilities

• Provide hands‑on personal care, including hygiene, toileting and daily living support.
• Perform delegated clinical tasks such as observations, simple dressings, specimen collection and eye care.
• Support patients with complex care needs within their home environment.
• Recognise and promptly escalate changes in patient condition to registered staff.
• Maintain accurate, confidential patient records.
• Assist with stock control, equipment use and general nursing team support.
• Build compassionate relationships with patients, families and multidisciplinary teams.

Required profile

• SVQ Level 3 in Healthcare or equivalent experience, or willingness to work towards it.
• Experience in health or social care settings.
• Ability to work independently as a lone worker.
• Excellent written and verbal communication skills.
• Full UK driving licence and willingness to travel across a wide area.
• Understanding of patient confidentiality, safety procedures and professional boundaries.
• Calm response to challenging situations and emergencies.
