Home Safe Support Worker – Part‑time (30 hrs/week)

About the role

Join the Home Safe Support team to deliver short‑term, reablement care that helps individuals regain independence after hospital stays or during periods of ill health. This permanent part‑time position involves visiting service users in their own homes across Northumberland.

Key responsibilities

• Provide short‑term, limited interventions to promote independence.
• Visit service users at home to support daily tasks, personal care and medication assistance.
• Assess need for low‑level equipment and arrange provision.
• Conduct risk assessments and develop, implement and review care plans.
• Maintain accurate, timely documentation and ensure compliance with legislation.
• Manage a caseload of service users and signpost to other professionals or services.
• Complete initial visits and home assessments.

Required profile

• Maintain confidentiality and treat each service user with dignity and respect.
• Valid UK driving licence and access to a car for work purposes.
• Ability to communicate clearly in spoken English.
• NVQ Level 3 or equivalent, or willingness to work towards it.
• Good general IT skills.
