About the role

We are seeking an experienced Occupational Therapist to work on a locum basis within a fast‑paced Discharge to Assess (D2A) service in Cheshire. The role involves supporting patients as they transition from hospital to their own homes, ensuring safe, timely discharges and promoting independence.

Key responsibilities

Conduct functional assessments in patients’ homes following hospital discharge.
Support safe discharge pathways and help reduce length of hospital stay.
Recommend and arrange equipment and minor home adaptations.
Collaborate with multidisciplinary team members including physiotherapists, social workers and community teams.
Develop patient‑centred care plans to maximise independence.

Required profile

HCPC‑registered Occupational Therapist.
Experience in community, discharge to assess or intermediate care settings.
Strong understanding of discharge planning and community rehabilitation.
Ability to work independently and manage a varied caseload.
Full UK driving licence (preferred) and access to a vehicle.
