Mental Health Support Worker – Burgess Hill

About the role

The DORS team is looking for a Mental Health Support Worker to support individuals during the first six weeks after discharge from hospital. You will work alongside a multi‑disciplinary team, providing person‑centred care in the community and helping to prevent re‑admission.

Key responsibilities

• Facilitate a person‑centred service that promotes independence.
• Complete dynamic risk assessments and develop support plans.
• Promote wellbeing and manage risk appropriately.
• Ensure compliance with company and national policies and procedures.
• Support people in their homes, facilitate safe discharges and avoid unnecessary admissions.
• Travel across West Sussex (travel expenses reimbursed, excluding commute).
• Collaborate with a diverse team of professionals.

Required profile

• Interest in the mental health sector and a desire to make a positive difference.
• Flexibility and adaptability to work in a fast‑paced environment.
• Ability to think on your feet and respond to changing client needs.
• Car owner/driver for travel within the Burgess Hill/Sussex area.
• Willingness to work Monday‑Friday with occasional weekend cover.
• Experience or qualifications in mental health support are desirable but not mandatory.
