Mental Wellbeing Practitioner

About the role

The Forward Trust is seeking a dedicated Mental Wellbeing Practitioner to join its team at HMP Chelmsford. In this role you will support adults with co‑existing mental health and substance misuse challenges, delivering both one‑to‑one and group interventions within a prison setting.

Key responsibilities

• Manage a caseload of individuals with complex mental health and substance misuse needs.
• Deliver tailored individual and group mental wellbeing interventions.
• Conduct assessments, develop care plans, provide advice, and refer to appropriate services.
• Build and maintain effective relationships with partner agencies and represent Forward Trust at multi‑agency meetings.
• Contribute to organisational and commissioner targets, focusing on measurable positive outcomes.
• Maintain accurate records in line with professional standards and organisational policies.

Required profile

• Compassionate, organised and resilient practitioner.
• Excellent communication and interpersonal skills.
• Experience supporting individuals with mental health and/or substance misuse needs.
• Confidence working collaboratively with a range of professionals.
