Non-Medical Prescriber – Kirklees Service

About the role

Change Grow Live is seeking an experienced Non‑Medical Prescriber (NMP) to join its high‑performing Kirklees service. You will work within a multidisciplinary team delivering person‑centred care to individuals facing complex physical health, mental health and substance‑use challenges.

Key responsibilities

• Conduct comprehensive clinical assessments and develop tailored care plans.
• Prescribe, review and optimise treatment in line with evidence‑based practice and national guidance.
• Support service users throughout their recovery journey, promoting independence and improved health outcomes.
• Respond to complex or urgent clinical situations, liaising with senior clinical colleagues as needed.
• Maintain accurate, timely clinical records in accordance with policy and legislation.
• Collaborate with colleagues, partner agencies and service users to deliver holistic care.
• Contribute to service development, clinical audit and appraisal processes.

Required profile

• Registered nurse with a current NMC PIN.
• Qualified Non‑Medical Prescriber with at least 3 years post‑qualification experience in community prescribing.
• Commitment to reducing health inequalities and delivering high‑quality care.
• Willingness to work flexible hours, including evenings when required.
