Nurse Associate – Primary Care

About the role

We are seeking a Nurse Associate to join our growing primary‑care team. Working across several GP practices, you will support the delivery of high‑quality, patient‑centred care alongside GPs, practice nurses and the wider multidisciplinary team.

Key responsibilities

• Conduct clinical observations and health assessments for patients.
• Run clinics and support long‑term condition management according to care plans.
• Provide wound care and perform basic clinical procedures.
• Offer health promotion advice, reassure patients and monitor concerns for escalation.
• Maintain accurate clinical documentation and help ensure smooth patient flow.

Required profile

• Experience working in GP practices or similar primary‑care settings.
• Ability to rotate between multiple practice sites and adapt to service demands.
• Commitment to patient safety, quality care and continuous professional development.
