Outreach Family Support Worker

About the role

The Outreach Family Support Worker provides high‑quality, early‑help support to children and families in Liverpool, using a whole‑family approach. Working on a temporary full‑time basis, you will act as the lead professional for complex cases, helping families access universal services and preventing escalation to statutory interventions.

Key responsibilities

• Take case ownership for level 2/3 early‑help referrals, complete assessments and arrange Team Around the Family meetings.
• Provide intensive, targeted support to vulnerable children and young people, including those with complex emotional or mental health needs.
• Assess risk and need, adopt the Lead Professional role, and coordinate multi‑agency plans using the Early Help Assessment Tool (EHAT) and Team Around the Family (TAF) processes.
• Develop and review Family Around the Family plans, ensuring timely implementation and monitoring.
• Apply de‑escalation techniques to manage challenging behaviour in adults and children.
• Use the Signs of Safety model and Family Outcomes Star to measure progress and outcomes.

Required profile

• Enhanced DBS clearance.
• Experience working with children, young people and families in a social‑care context.
• Ability to lead multi‑agency teams and manage complex caseloads.
• Strong communication and relationship‑building skills.
