Outreach Support Worker – Adult Care (Full‑time)

About the role

We are seeking compassionate individuals to join our team as Outreach Support Workers, providing person‑centred care to adults with learning disabilities and complex needs across South Ribble. The role is based in the client’s home, allowing you to build meaningful relationships while supporting independence.

Key responsibilities

• Deliver 1:1 support in the client’s home, assisting with daily living activities.
• Provide personal care, including hygiene and application of prescribed creams.
• Encourage independence and participation in community outings and social activities.
• Manage challenging behaviours and support individuals with forensic risk backgrounds safely.
• Complete scheduled sleep‑in shifts (approximately 1–2 per week) as required.

Required profile

• Caring, reliable and compassionate nature.
• Strong communication skills and commitment to high‑quality, person‑centred care.
• Willingness to work alternate weekends and flexible shift patterns.
