Part-Time Primary Care Pharmacist – Medicines Optimisation

About the role

We are looking for a part‑time Primary Care Pharmacist to join the Brooklands Medical Practice team in Manchester. The role focuses on medicines optimisation, supporting prescription‑related queries and delivering clinical medication reviews as part of a multidisciplinary team dedicated to patient wellbeing.

Key responsibilities

• Respond to prescription‑related enquiries from clinicians and patients.
• Conduct comprehensive clinical medication reviews for primary‑care patients.
• Collaborate with doctors, nurses and other healthcare professionals to optimise therapeutic outcomes.
• Document interventions and recommendations in the patient record.
• Contribute to the development of medicines optimisation strategies within the practice.

Required profile

• Registered pharmacist with at least two years post‑registration clinical experience.
• Strong communication skills and ability to work within a multidisciplinary team.
• Commitment to patient‑centred care and continuous improvement.
