Part-time Social Worker – Review Team (3 days/week)

About the role

The Hospital Assessment and Community Review (HACR) Service in Havering is seeking a qualified Social Worker to join its Review Team on a part‑time basis (three days per week). You will support residents transitioning from hospital discharge to independent living, ensuring they receive appropriate care and rehabilitation services.

Key responsibilities

• Conduct Care Act assessments for individuals following a Home First pathway.
• Review and update care and support plans to meet changing needs while promoting wellbeing, choice, independence, and safety.
• Support residents across various rehabilitation pathways, including non‑weight‑bearing, specialist, slow‑stream, and the Collar and Brace After‑care Service (CBACS).
• Collaborate with Havering’s Quality Assurance Team to address concerns in care homes or domiciliary providers and safeguard residents.

Required profile

• Fully qualified Social Worker registered with Social Work England (SWE).
• Commitment to person‑centred practice and multidisciplinary teamwork.
• Strong assessment and care‑planning abilities.
