PATCH Nurse – Paediatric Hospital-at-Home

About the role

The PATCH Nurse will join an innovative Paediatric Hospital-at-Home service, delivering high‑quality assessment and treatment to acutely unwell children in the comfort of their own homes. Working closely with the NWL PATCH Team, Virtual Wards Programme and West London Children’s Healthcare, you will provide compassionate, timely care while supporting the wider hospital network.

Key responsibilities

• Independently assess and manage acutely unwell children referred to the PATCH team.
• Deliver face‑to‑face, virtual and telephone consultations as appropriate.
• Escalate care to hospital teams when required, ensuring seamless continuity.
• Maintain accurate clinical records and adhere to NHS safety and quality standards.
• Travel to patient homes using public transport and operate safely as a lone worker.

Required profile

• Registered Nurse with paediatric experience.
• Ability to work autonomously and make clinical decisions.
• Willingness to undertake a six‑month probationary period.
• Commitment to the NHS values of equality, diversity and patient‑centred care.

Required skills

• Valid driving licence.
• Capability to work alone and travel by public transport.
• Proficiency in conducting face‑to‑face, virtual and telephone consultations.
