Physiotherapist – Home Teams (Stewartry)

About the role

This permanent position offers an experienced Physiotherapist the chance to join a multidisciplinary Home Teams service in Stewartry. You will work within a community‑focused environment, helping adults with frailty and complex health needs maintain independence and avoid unnecessary hospital admissions.

Key responsibilities

• Deliver specialist physiotherapy assessment, treatment and ongoing management for adults across community and acute interfaces.
• Manage a complex caseload, supporting safe transitions from hospital to home and reducing readmission risk.
• Coordinate cases, promote health, conduct audits and provide expert advice on professional interventions.
• Supervise and mentor less experienced staff, support workers, students and external agencies.
• Provide physiotherapy leadership within the Home Teams, collaborating with other professional leads.

Required profile

• Qualified physiotherapist with several years of clinical experience in community or acute settings.
• Ability to work autonomously and make person‑centred decisions.
• Strong communication skills for multidisciplinary teamwork.
• Commitment to service improvement and professional development.
