Placement & Brokerage Officer

About the role

The Placement & Brokerage Officer works within the Care Arrangement Team to coordinate and arrange care packages for individuals with assessed needs. The role ensures timely access to appropriate support, liaising with community care providers and adhering to the Care Act 2014.

Key responsibilities

• Arrange and record care packages accurately using the designated case management system.
• Manage daily demand for care and support, prioritising urgent cases such as hospital discharges.
• Monitor market capacity and trends, reporting any issues affecting provider availability.
• Conduct quality assurance checks on brokerage referrals to resolve errors or inconsistencies.
• Provide advice to health and social care professionals on arrangement processes and market options.
• Negotiate rates in line with agreed Wirral rates and escalate exceptions for approval.
• Support out‑of‑area placements, ensuring continuity of care and policy compliance.
• Build and maintain relationships across multi‑agency, voluntary and community networks.

Required profile

• Experience working in adult social care or a related health‑care environment.
• Knowledge of the Care Act 2014 and statutory requirements for care provision.
• Strong organisational skills with the ability to manage competing priorities.
• Effective communication and stakeholder‑engagement abilities.
