Practice Nurse – Part‑time (20 hrs/week) – London Clinics

About the role

GP at hand is seeking enthusiastic Practice Nurses to join its digitally enabled NHS General Practices across Central London. The role is part‑time (20 hours per week) and combines face‑to‑face clinics, remote consultations and desktop reviews to deliver high‑quality, patient‑centred care.

Key responsibilities

• Deliver evidence‑based nursing care in line with NMC standards.
• Run clinics covering diabetes, hypertension, cardiovascular disease, cervical screening, NHS health checks, immunisations, travel health assessments and wound care.
• Perform venepuncture, dressings, and removal of clips or sutures.
• Conduct remote consultations and complete desktop clinical reviews, including long‑term condition management and QOF‑related tasks.
• Identify and escalate safeguarding concerns and maintain accurate electronic patient records.
• Support population health initiatives and digital pathway development.

Required profile

• Registered with the Nursing and Midwifery Council (NMC) and qualified as a Practice Nurse.
• Experience in primary‑care or community nursing settings.
• Ability to work a flexible shift pattern between 8:00 am and 8:00 pm across multiple London sites.
• Commitment to patient safety, quality improvement and multidisciplinary collaboration.
