Qualified Social Worker – Adults & Communities (Promoting Independence)

About the role

The Adults & Communities department of Leicestershire County Council is seeking a qualified Social Worker to join the Promoting Independence team. Working within a hybrid model, you will support adults to assess their needs, manage personal budgets and develop sustainable support plans.

Key responsibilities

• Conduct person‑centred assessments and co‑create support plans that align with individual budgets and outcomes.
• Coordinate complex, high‑risk cases, acting as the primary care coordinator for clients with trauma, challenging circumstances or specialist needs.
• Investigate and manage safeguarding concerns, adult mental capacity assessments and Continuing Healthcare referrals.
• Promote independence by leveraging natural support networks, community resources, universal services and Direct Payments.
• Collaborate with health professionals, local providers, emergency services and voluntary agencies to deliver integrated care and facilitate hospital discharges.

Required profile

• Recognised degree in Social Work (CQSW or DipSW acceptable).
• Active registration with Social Work England (or HCPC equivalent).
• Enhanced DBS clearance for regulated activity.
• In‑depth knowledge of the Care Act 2014, Mental Health Act, Mental Capacity Act and safeguarding legislation.
