Qualified Social Worker – Discharge to Assess

About the role

We are seeking a qualified social worker to join Bristol City Council’s Discharge to Assess team. The role involves assessing and reviewing individuals after hospital discharge, developing care and support plans, and contributing to funding decisions.

Key responsibilities

• Conduct post‑hospital discharge assessments and reviews.
• Develop and implement Care and Support Planning (CAA) and Best Interest (BI) decisions.
• Apply Mental Capacity Act (MCA) principles in case work.
• Provide safeguarding duty cover as required.
• Attend Quality Assurance panels to support funding provision.

Required profile

• Qualified social worker registered with the appropriate professional body.
• Willingness to register with Remedy Recruitment for locum assignments.
• Ability to undergo an Enhanced Disclosure check via DBS.
• Commitment to safeguarding and promoting the welfare of vulnerable people.

Required skills

• Knowledge of Care and Support Planning, Best Interest decisions, Mental Capacity Act and safeguarding procedures.
