Qualified Social Worker

About the role

We are seeking a qualified Social Worker to join an 18‑bed women’s Level 2 rehabilitation inpatient service in Cullompton, Devon. You will support patients as they progress through recovery, helping them achieve greater independence and a successful transition back to the community.

Key responsibilities

• Manage a complex caseload and complete comprehensive social care assessments.
• Conduct risk assessments and statutory reporting, including CPA/MH requirements.
• Support safeguarding processes and internal investigations.
• Collaborate with families, Local Authorities, and external agencies.
• Contribute to multidisciplinary team meetings and care planning.
• Facilitate access to welfare rights, housing, advocacy, and community support services.
• Assist with discharge planning and setting future goals for patients.
• Work closely with clinical teams to deliver high‑quality, person‑centred care.

Required profile

• Social Work degree and registration with Social Work England.
• Previous experience in a similar healthcare environment supporting vulnerable adults.
• Strong understanding of safeguarding procedures and social care assessments.
