Qualified Social Worker – Learning Disability & Early Adulthood (Hybrid)

About the role

We are seeking a qualified Social Worker to join the Learning Disability and Early Adulthood Team within Stockport Council. This is a contract position paying £30 per hour and offers a hybrid working model with a minimum of two days in the office.

Key responsibilities

• Review and coordinate complex care packages for adults with learning disabilities, autism, mental health needs and other related conditions.
• Assess and arrange joint funding arrangements across Early Adulthood, Learning Disability, Mental Health, Autism and Community Health Care services.
• Collaborate with multi‑agency partners to ensure integrated support and compliance with statutory frameworks.
• Maintain accurate case records and produce high‑quality reports for internal and external stakeholders.
• Contribute to team meetings, case reviews and service development initiatives.

Required profile

• Qualified Social Worker with current registration.
• Demonstrated experience reviewing complex packages and joint funding in the specified sectors.
• Ability to work effectively in a hybrid model, attending the office at least two days per week.
• Strong analytical and communication skills to liaise with diverse professionals.
