Reablement Assistant – Dementia Care Unit

About the role

We are seeking a compassionate Reablement Assistant to join a dedicated Dementia Care Unit in Liverpool. You will support adults living with dementia, helping them maintain dignity, independence and wellbeing in a safe, person‑centred environment.

Key responsibilities

• Provide personal care, mobility assistance, meal preparation and hydration support.
• Encourage residents to engage in meaningful activities and retain independence where possible.
• Build trusting relationships and offer emotional reassurance to individuals experiencing memory loss.
• Collaborate with nurses and the multidisciplinary team to deliver high‑quality dementia care.
• Follow and contribute to individual care plans, maintaining accurate records and reporting changes promptly.
• Uphold council policies on safeguarding, equality, dignity and anti‑discriminatory practice.

Required profile

• Experience in dementia, residential or similar health and social care settings.
• Compassionate, patient and person‑centred approach.
• Strong communication and interpersonal skills.
• Ability to work effectively within a multidisciplinary team.
• Reliability, flexibility and commitment to high‑quality care.
