Reablement Worker

About the role

Northumberland County Council is seeking a Reablement Worker to join its Short‑Term Support Service in the West area, based at Dene Park House, Hexham. This is a permanent, part‑time position (30 hours per week) focused on helping adults regain independence after illness, hospital discharge or to avoid unnecessary admissions.

Key responsibilities

• Visit clients in their own homes to provide short‑term, goal‑oriented support.
• Assist with personal care tasks such as showering and dressing.
• Support medication management, meal preparation and household chores.
• Improve client mobility within the home and encourage safe community outings.
• Help with shopping, laundry and other daily living activities.
• Work a rotating roster covering AM and PM shifts, 7 am‑10 pm, seven days a week.

Required skills
