RECONNECT Peer Support Worker

About the role

The Forward Trust is seeking a RECONNECT Peer Support Worker to deliver a tailored, inclusive service for individuals preparing to leave custody in Bedfordshire and Buckinghamshire. Working across HMP Bedford, HMP Woodhill, Yarl’s Wood IRC and the wider community, you will use your lived experience to help clients access health, wellbeing and substance‑use support.

Key responsibilities

• Process referrals received up to 12 weeks before release and conduct thorough assessments.
• Create bespoke care plans and provide up to six months of post‑release support.
• Manage a diverse caseload, facilitating reintegration and engagement with community health services.
• Assist the Team Leader in recruiting and training prison peer supporters.
• Build and maintain partnerships with external agencies, families, prison staff and healthcare providers.
• Maintain accurate case notes, risk assessments and administrative records.
• Make referrals to Forward Trust services such as Meet and Greet and Meet at the Gate.

Required profile

• Lived experience of physical or mental ill health, substance misuse, or the criminal justice system.
• Strong empathy and ability to build trusting relationships with service users.
• Commitment to equality, diversity and inclusion.
• Ability to work both independently and as part of a multidisciplinary team.

Required skills

• Experience conducting assessments and developing personalised care plans.
• Knowledge of NHS‑commissioned health and wellbeing pathways.
• Proficiency in case‑note documentation and risk‑assessment processes.
