Relief Support Worker – Acquired Brain Injury Service

About the role

The Relief Support Worker will join a newly developed Acquired Brain Injury service in Chorley, providing personalised supported living for a client transitioning from hospital to community life. This temporary, full‑time position focuses on empowering the individual to achieve independence, engage with family and community, and pursue personal interests.

Key responsibilities

• Deliver person‑centred support across daily living activities and community participation.
• Assist the client in developing independent living skills such as budgeting, cooking, and personal care.
• Facilitate engagement in hobbies including fitness, video games, music, puzzles, bowling, cinema, football, snooker and socialising.
• Work flexibly between the Chorley base and the client’s current placement to ensure smooth service development and transition.
• Maintain accurate records and communicate progress to the multidisciplinary team.

Required profile

• Minimum 12 months experience in a support or care role.
• Demonstrated ‘can‑do’ attitude and reliable, person‑centred values.
• Eligibility to work in the UK and residence within the country.
