Remote Primary Care Pharmacist – Full/Part Time

About the role

OneMedical Group is seeking an experienced Primary Care Pharmacist to provide remote pharmaceutical support to Minehead Medical Practice in Somerset. The role can be full‑time (40 hrs) or part‑time (minimum 20 hrs) and will involve virtual patient consultations, medicines optimisation and collaboration with the multidisciplinary team.

Key responsibilities

• Deliver remote patient services, including medication reviews, repeat prescription advice and general pharmaceutical queries.
• Provide virtual consultations via phone or video, offering education and optimisation of medicines.
• Support prescribing activities, conduct audits and manage high‑risk drug safety.
• Assess patients’ knowledge and socio‑economic factors to tailor advice.
• Prescribe urgent medications using summary care records where appropriate.
• Identify and manage clinical risks, ensuring patient safety.
• Act on child or adult protection concerns following protocol.
• Collaborate with the multidisciplinary team on service delivery and quality improvement.
• Record accurate information in clinical systems and electronic databases.
• Uphold GPhC standards of ethics, conduct and performance.

Required profile

• Registered with the General Pharmaceutical Council (GPhC).
• Completed or scheduled to complete the CPPE Pathway this year.
• Extensive experience working in a primary‑care pharmacy setting.
• Ability to work independently and make professional decisions remotely.

Required skills

• Proficiency with clinical systems.
• Experience using electronic patient records and summary care records.
