Senior Case Manager

About the role

The Senior Case Manager will support adults and children rebuilding their lives after life‑changing neurological injuries. Working autonomously and in partnership with multidisciplinary teams, you will coordinate rehabilitation programmes, ensure person‑centred care and help clients achieve greater independence.

Key responsibilities

• Conduct comprehensive assessments and develop goal‑focused rehabilitation plans.
• Coordinate multidisciplinary teams, external professionals and support services.
• Recruit, supervise and support care teams, identifying training needs.
• Produce high‑quality clinical documentation, reports and case records.
• Monitor progress, risk, budgets and outcomes while advocating for clients.
• Contribute to clinical governance, research, audits and service improvement.

Required profile

• HCPC registration with an Occupational Therapy or Physiotherapy qualification and a neurological specialism.
• Proven experience managing complex neurological rehabilitation caseloads.
• Strong knowledge of clinical governance, rehabilitation legislation and multidisciplinary working.
• Full UK driving licence and willingness to travel and join an on‑call rota.
