Senior Clinical Fellow – Community Care of the Elderly

About the role

The Aneurin Bevan University Health Board is seeking a Senior Clinical Fellow to join its Community Care of the Elderly team in Torfaen. You will provide senior clinical input to community wards, working closely with a multidisciplinary team to deliver safe, person‑centred care for older adults.

Key responsibilities

• Deliver senior clinical assessment and management for elderly patients in community inpatient settings.
• Lead discharge planning and coordinate with nursing, therapy, pharmacy and social care colleagues.
• Support collaborative decision‑making across the multidisciplinary team.
• Contribute to the development of clinical protocols and quality improvement initiatives.
• Provide mentorship and teaching to junior doctors and trainees.

Required profile

• Qualified medical doctor with experience in elderly care or general medicine.
• Demonstrated ability to work autonomously and lead clinical teams.
• Commitment to delivering person‑centred, evidence‑based care.
• Willingness to engage in continuous professional development.
