Senior Social Work Practitioner – Mental Health

About the role

The client, Caerphilly County Borough Council, is seeking a Senior Social Work Practitioner to join its Community Mental Health Team. You will work within a multidisciplinary setting to deliver care and treatment planning for adults requiring community‑based health and social support, while upholding independence, dignity and recovery.

Key responsibilities

• Assess individual needs and coordinate care plans for service users and their carers.
• Conduct risk assessments and arrange services to minimise and manage risk.
• Provide social work services in line with mental health legislation and guidance.
• Monitor and evaluate the effectiveness of care plans, ensuring user and carer satisfaction.
• Collaborate with other agencies and networks to deliver coordinated assessments.
• Act as an Approved Mental Health Professional (AMHP) when required.
• Identify unmet needs and contribute to service development and integration with local health boards.

Required profile

• Qualified social worker with extensive experience in adult mental health.
• Demonstrated ability to work autonomously and lead less‑experienced staff.
• Strong understanding of the Mental Health Act and AMHP responsibilities.
• Excellent multidisciplinary teamwork and communication skills.
