Social Worker – Adults Care & Support

About the role

Join Northumberland County Council as a Social Worker delivering care and support for adults aged 18 and over. You will work within the Adults, Ageing & Wellbeing Directorate, providing holistic, integrated services that promote independence, health and wellbeing for vulnerable adults across the county.

Key responsibilities

• Conduct comprehensive assessments of adults with physical and mental health needs.
• Develop, implement and review person‑centred care plans.
• Coordinate support with partner agencies such as health services, housing, police and drug‑and‑alcohol teams.
• Manage a caseload autonomously while adhering to the Care Act and statutory duties.
• Provide regular reports and maintain accurate case records.

Required profile

• Qualified Social Worker (post‑qualification experience or successful completion of the ASYE programme).
• Commitment to continuous professional development.
• Strong understanding of the Care Act and local authority duties.
• Ability to work independently with supervision from senior practitioners.
