Social Worker – Children with Disabilities

About the role

Join Herefordshire Council's Children with Disabilities team as a Social Worker, supporting children and young people with additional needs and their families. You will deliver person‑centred assessments, coordinate multi‑agency services and champion the voices of children to help them thrive.

Key responsibilities

• Conduct holistic assessments of children with disabilities and develop tailored care plans.
• Coordinate and monitor services across health, education and social care partners.
• Build trusting relationships with families, ensuring they are involved in decision‑making.
• Promote independence and community participation for children.
• Provide guidance to junior staff and contribute to team reflection and learning.

Required profile

• Registered with Social Work England with a recognised qualification (Degree/DipSW/CQSW).
• Minimum three years post‑qualifying experience working with children with disabilities.
• Strong commitment to collaborative, inclusive practice.
• Resilient and reflective when handling complex cases.
