Social Worker – Integrated Triage & Assessment (Ceredigion)

About the role

This position is for an Integrated Triage & Assessment Social Worker supporting Ceredigion Council. The role can be based from home but requires regular visits across the county to act as the first point of contact for all referrals and enquiries.

Key responsibilities

• Gatekeep all electronic and Clic referrals, gathering additional information as needed.
• Decide on next steps: escalation to safeguarding, de‑escalation, or assignment to appropriate assessment teams.
• Provide information and advice to families to promote self‑resilience when no further action is required.
• Conduct proportionate assessments using the Signs of Safety model, identifying concerns, strengths and agreeing on resolution plans.
• Undertake whole‑family assessments and develop recovery plans for adults to regain independence.

Required profile

• Relevant Social Work qualification.
• Registration with Social Care Wales (or Social Work England) and an enhanced DBS on the update service.
• Minimum three years of relevant experience.
• Driving licence with access to a car.

Required skills

• Proficiency with the Signs of Safety assessment model.
