Social Worker – Older Adults & Disabilities (Leicestershire)

About the role

We are looking for a qualified Social Worker to join the Cognitive & Physical Disabilities team supporting older adults in the Charnwood/Loughborough area. The role combines office‑based duties with field visits and offers a hybrid working pattern.

Key responsibilities

• Conduct Care Act assessments and develop support plans for older adults with frailty, dementia, stroke, Parkinson’s disease, brain injury or physical disabilities.
• Carry out safeguarding enquiries and manage complex family situations.
• Perform mental capacity assessments, best‑interest decisions and continuing healthcare reviews.
• Participate in duty rota, attend Court of Protection hearings and collaborate with multidisciplinary teams.
• Deliver visits and meetings in the local area while maintaining accurate case records.

Required profile

• Qualified Social Worker, fully registered with Social Work England.
• Significant post‑qualified experience in adult social care.
• Extensive background working with older adults and cognitive/physical disability cases.

Required skills

• £34 per hour (umbrella rate).
• Hybrid working – office base with remote days when not on visits.
