Support Coordinator – Person‑Centred Care

About the role

Creative Support Ltd is looking for an experienced Support Coordinator to lead a dedicated team delivering high‑quality, person‑centred care for individuals with learning disabilities, physical disabilities and additional mental health needs. You will ensure services are tailored to promote independence and meaningful lives.

Key responsibilities

• Supervise and coach a team of care staff, ensuring delivery of outcome‑focused support.
• Coordinate care plans in partnership with families, professionals and multi‑disciplinary teams.
• Deputise for the Service Manager as required.
• Promote and monitor the wellbeing and independence of service users.

Required profile

• Relevant experience in social care supporting people with learning or physical disabilities and mental health needs.
• Minimum NVQ Level 3/H&SC Diploma or equivalent; willingness to train to Level 5.
• Excellent verbal and written communication, organisational and collaborative skills.
• Flexibility to work shifts, weekends and occasional sleep‑in duties.

Required skills

• Understanding of person‑centred care principles.
• Ability to liaise with Care Managers and multidisciplinary teams.
