Support Worker – Community Care

About the role

We are looking for a compassionate Support Worker to join our Outreach Team in Devizes and the surrounding area. You will help individuals with learning disabilities, autism, mental health challenges and complex needs live as independently as possible, supporting them to access community activities, develop life skills and achieve personal goals.

Key responsibilities

• Encourage service users to take an active role in their community and promote independence.
• Assist with daily living activities, including personal care, meal preparation, budgeting and routine planning.
• Communicate effectively using each person’s preferred method.
• Motivate and engage individuals to enhance their quality of life.
• Provide positive behavioural support where required.
• Maintain accurate records and report any changes in health or circumstances.
• Work collaboratively as part of a team while also using your own initiative.

Required profile

• Enthusiastic, caring and patient with a proactive approach.
• Excellent people skills and the ability to build rapport.
• Minimum 12 months’ right to work in the UK.
• Availability for full‑time, part‑time or bank shifts.
